2007-08 Oregon School Directory - Information Update Request
Summary: ACTION REQUIRED -- We need your help to prepare the new edition
of the Oregon School Directory. The directory is widely used by
educators
and the public to quickly and easily locate information about our public
schools. In order for us to make sure that information is accurate,
please let us know the changes in your district's listings.
*	Deadline:  FAX (503-378-5156) or email your information no later
than June 29 to gene.evans at state.or.us.
*	Check our website for a PDF of the latest information we have
for your district (http://www.ode.state.or.us/pubs/update/6dist.pdf) 
*	Corrections/revisions: Please carefully review your ESD/School
District information at http://www.ode.state.or.us/pubs/update/6dist.pdf
and e-mail changes to gene.evans at state.or.us. Type the changes only and
include old information and new information along with effective dates.
*	Please do not retype all of the information (retyping requires
us to proofread your information against ours to determine the changes).
Or on a copy, you can mark all changes or additions you wish to appear
in the 2007-08 directory.
*	New/closed schools, school name changes - Provide an electronic
copy of the district board minutes where action was taken to open a new
school, close an existing school, or change the name of an existing
school.
*	Address (include physical address) - Please include the physical
(street) address in addition to your mailing address. Include the "+4"
digits for your zip code. (The Post Office requires us to use the Zip+4
and charges a higher rate of postage when not used.) The URL for the
Zip+4 Lookup is http://zip4.usps.com/zip4/welcome.jsp. 
*	Phone and fax numbers - Include phone and fax numbers not
previously listed.
*	E-mail addresses - Please include an e-mail address for the
superintendent and principal for each district and school. 
*	Staff titles - Notify us of staff who have left as well as new
staff (include start dates for new staff and end dates for staff
leaving).
*	Limit the number of staff in an area to one. Exceptions are
Superintendent, Deputy Superintendent, and Principal, which can have
more than one. If you list more than one person as Superintendent,
Deputy Superintendent, and/or Principal, they will be printed in
alphabetic order in the directory. 
Use only the following titles:

Superintendent or Interim Superintendent
Service Center Administrator (ESDs)
Deputy Superintendent
Assistant Superintendent
Deputy Clerk
Administrative Assistant
Business Manager
Human Resources/Personnel 
Communications
Curriculum Instruction 
Special Education

Assessment
Special Services
Technology
Media/Library
Activities
Nutrition
Transportation
Safety
Facilities

School staff
Principal or Interim Principal or Head Teacher or Co-principal or
Director
*	Grades - Submit the lowest grade and highest grade offered by
each of your schools.
*	Contact-Please indicate a person we should contact if we have
questions regarding your information.
