Some may question the necessity of keeping your personal information current. We assure you that having your personal information current is necessary for the Board to meet our responsibilities to all of you: issuing licenses/certifications and renewals, mailing renewal reminders, responding to questions, and maintaining the nursing assistant registry and OSBN data base. You have the option of going to the Boards web page, www.oregon.gov/osbn, go to the header "Other Online Services" establish a password and you can easily change your address and employer information when required. You can also view your personal information page and update your continuing education information. Or, you can send a written request to change your address or employer information. Name changes always require a written notification and legal proof of the name change. Find a form for personal information changes on our web page under "Forms". The licensed nurse can find their rules regarding name, address, and employer changes in the Nurse Practice Act, Division 31 at OAR 851-031-0090. The certified NA or MA can find their rules regarding name, address, and employer changes in the Nurse Practice Act, Division 62 at OAR 851-062-0120.
