[collaborative-vr] Agreement Statement/Documentation

Hi All,

My org is asking if I can create an agreement statement for our libraries
to sign to participation in AskMN statewide program. We haven't done this
before as our libraries are not billed for participating. However, they
would like to start doing.

Do you have an agreement to participate-type statement or document you have
libraries sign as they join your program? Do you other policies of
participation (not training docs) that are shared with your libraries? If
so, can you share them with me?
