Good morning.  The 503-603-7626 fax number for PERS is currently not
working.  It is receiving some information but is storing it because
there is a broken part.

 

Agencies fax beneficiary forms directly to PERS, etc.  Please hold your
faxes to them until we notify you that the issue has been resolved.

 

Our fax machine is still working so please go ahead and send all other
forms you normally send to us and we will continue processing them.
