[DAS_Centralized_PERS_Services_Team] PERS Beneficiary Forms Notification

Good morning.  We just want to notify agencies that PERS has had some recent difficulties with not receiving timely beneficiary forms and changes to beneficiary forms for employees.



Although agencies may accept a completed form for their employee it still remains the employee's responsibility to make sure a change of beneficiary designation is received by PERS. A change of beneficiary designation is not considered effective until the form is stamped as received by PERS.



So this is a reminder of what is needed for all members:



Tier 1 and Tier 2 Members:



-          IAP: Pre-Retirement Designation of Beneficiary Packet; AND

-          Pre-Retirement Designation of Beneficiary Packet

OPSRP Members:


-          IAP: Pre-Retirement Designation of Beneficiary Packet.

These forms can be found on the PERS website at the link below:

Once the form(s) have been completed please fax them to 503-598-0561 or mail them to the following address:

Please do not send either of these two forms to our address or fax number.  These forms contain confidential information so please handle them accordingly.
