The CPERS fax machine has been shut off. Please see the attached email for the current status check process. 

If you need to reach CPERS for anything else, please email Central.PERSServicesTeam at oregon.gov. You may send paperwork in via email as long as it does not contain social security numbers. Please use the employee's OR# in place of SSN on any documents, such as the USERRA form.
