[DAS_Centralized_PERS_Services_Team] Status Checks Alternate Fax Number

Regarding PERS status checks- we've been informed that PERS is experiencing technical difficulties with their fax number designated for status checks (503-603-7626). They are working to resolve the issue. In the meantime, an alternative fax number has been provided for agency use: FAX 503-968-5791.

Remember, you may also request a status check via phone: PHONE 1-888-320-7377.

We will let you know when the primary number has been restored.
