PERS is experiencing an outage with their status check phone line. For the time being, please send the status check request form to DAS.Status.Checks at PERS.oregon.gov<mailto:DAS.Status.Checks at PERS.oregon.gov> . Please do not include the SSN on the form when sending emails.

PERS is working to resolve the phone outage and we will send another email once resolved.
