Payroll and HR Partners,

We're asking each of you to take a few minutes to update your agency contact info for PERS related communications.

Please use the attached form to indicate the appropriate person for each unit, and their contact information- be sure to fill in the top portion to include Agency name, Agency # and PERS Employer #.

We ask that you complete this by Apr,9, 2022 and email the form to me at Coleen.Hogan at das.oregon.gov<mailto:Coleen.Hogan at das.oregon.gov>.

When emailing, please include your agency number in the subject line of the email to ensure proper updates are made.
