We're asking each agency to review and update their agency contact information for PERS related communications.

Please use the attached form to indicate the appropriate person for each unit, and their contact information- be sure to fill in the top portion to include Agency name, Agency # and PERS Employer #.

Please include your agency number in the subject line of the email to ensure proper updates are made.
