RE:      Survey - Services for Students who are Blind/Visually Impaired

 

Attached is an online survey, asking for your input and perceptions
about statewide services for children who are blind/visually impaired.
Information provided by parents/family members, school districts,
teachers and other service providers, and community members will be used
to inform services provided by Oregon's school districts, Regional
Programs, and the Oregon School for the Blind (OSB). This information
also will be used to follow up on discussions about co-location of the
School for the Blind and the School for the Deaf. 

 

This survey does not require any submission of personal information.
There are 6 questions and should requires 10 minutes to complete. If
anyone has problems accessing the survey, or would like to receive a
print copy, large print copy, and/or Braille copy please contact Colleen
Carson at 503-378-3826. A different survey will be disseminated to
parents next week.

survey. If there are any problems linking directly, respondents can "cut
and paste" the URL address into Internet Explorer or other web browser.
This survey will be open for approximately 2 weeks, closing on October
24.
