[HealthyTribes] National Diabetes Prevention Program - Outreach to CCOs

Hello TPEP and Healthy Communities Grantees,

As you may be aware, HPCDP and partners throughout the state have been working to establish and expand access to the National Diabetes Prevention Program (DPP). You can learn more about the DPP here: http://public.health.oregon.gov/PreventionWellness/SelfManagement/Pages/index.aspx#DPP

As part of these efforts, starting in mid-May Don Kain of OHSU's Harold Schnitzer Diabetes Health Center will be contacting each of Oregon's Coordinated Care Organizations to assess if they currently have any protocols or practices in place to refer people with prediabetes to a DPP, and to offer technical assistance on identifying people with prediabetes and setting up DPP referral systems. As a DPP Master Trainer as well as a Registered Dietician and Certified Diabetes Educator, Don brings a wealth of knowledge and experience related to diabetes prevention.

For SRCH grantees: We'll be in touch separately with SRCH coordinators with more information on this as relates to SRCH grant activities.
