The result is the "Healthy Active Oregon: Statewide Physical Activity and Nutrition Plan 2007-2012" (the PAN Plan). Many of you may have contributed to the creation of this Plan. Today, the PAN Plan Steering Committee is sending you this survey to get your input and feedback about prioritizing the objectives included in the PAN Plan. Please complete this survey to share your priorities for physical activity and nutrition in Oregon. The priorities identified through this process will provide us with a roadmap for developing a one-year operations plan with targeted strategies to accomplish our priority objectives. We appreciate your commitment to increasing physical activity and healthy eating in your communities. We value all the work in your communities to reach the overarching goals to prevent obesity and chronic diseases in Oregon.
