============
The Transition Coalition (www.transitioncoalition.org
<http://www.transitioncoalition.org/> ) at the University of Kansas
(www.ku.edu <http://www.ku.edu/> ) is identifying websites related to the
transition from school to adult life for youth with disabilities.
The Transition Coalition provides online training for various groups
involved in transition. We would like your assistance in disseminating
What's Working Well With Transition: Models of Success materials.
Through our What's Working Well With Transition: Models of Success project,
we are soliciting successful stories, strategies and/or programs from people
with disabilities, their families and professionals who either provide
transition services or who experience it daily.
Your assistance will help us gather great stories, successful strategies,
exemplary programs and best practices from different stakeholders.
Some ways you can help us distribute our nomination materials include:
Through your website (linking to our website)
Through any appropriate email list-serves you may have or receive
Through any newsletters (print or electronic) your organization may publish
By sending print versions of the Nomination forms to your organization's
mailing lists
Attached to this message is a zipped folder including:
* An explanation of the items included in the folder
(ListofMaterials.doc)
* PDF's of the Two types of What's Working Well With Transition
nomination brochures, a full version and an Abridged version, which was
designed specifically for people who may need print accommodations
* An advertisement that might be appropriate for a newsletter or
magazine
* A document with a JPEG of the logo and information we would like you
to put on your site, as well as instructions for linking to our site.
Please feel free to disseminate this information far and wide!
If you have questions, have difficulty opening the zip, or if you would like
all of the materials mailed to you on a CD & in print format, please reply
to this e-mail (modelsofsuccess@ku.edu) or call 785/864-0686 and ask about
Models of Success.
Thank you for assisting us in this effort.
