The Schools and Libraries Division (SLD) recently posted new guidance on
the use of the revised versions of the Form 486 and the Form 479 with a
special emphasis on the new CIPA requirements for libraries and library
consortia. (See

All applicants should note that the revised version of the Form 486 (dated
August 2003) is now the only acceptable version.  The online version of the
Form 486 became temporarily inoperative on August 14th when the new
version became E-rate message effective.

The new online version of the form is not expected to become operative
until late September. Any applicant wishing to file a Form 486 now, must
download a copy of the new version (available at 
http://www.sl.universalservice.org/form/default.asp#applicants),
fill it out by hand and mail it to the SLD.
