[Libs-Or] Faxed SRP evaluations

I just noticed that our fax machine broke while I was away and we currently do not have one! So if you faxed your SRP evaluation to me in the last 2 weeks (to be on the safe side) please assume I DID NOT receive it. If you could resend it to me at: 541.389.2982 and make sure it is very clearly marked to my attention. For anyone who has not sent theirs in yet, please use either email, snail mail or the above fax number, NOT the one on the evaluation form. Thanks for your cooperation,
