[Libs-Or] SRP evaluation forms

Hi everyone:

It is time once again to fill out summer reading evaluation forms.

It is important that we have this information for the purpose of
obtaining grants and other funding in the future.

Us CSD officers are kind of mean and we have been known to withhold SRP
manuals until we have received an evaluation from the previous year.
We'll do it again if we have to, but that's kind of a pain for everyone
concerned, so please just take a few minutes and do it by September 30,
2005.

The CSLP manual contains an evaluation form, but please do NOT use that 
one.  Instead, please fill out the form contained in the attached Word 
document.

You can either e-mail, fax or snail mail it back to me.  The form should 
also soon be available on the CSD web site at www.olaweb.org/CSD.
