[Libs-Or] Cover Oregon - paper applications

While the bugs are being worked out on the Cover Oregon web site, citizens can submit paper applications. It is still recommended that they work with certified insurance agents and community partners<http://www.coveroregon.com/agent/find>, however PDF forms<https://apps.state.or.us/mbs/landing.jspx> are available directly from the web site.

Cover Oregon staff recommend that forms be printed out and faxed or put in the mail rather than submitted electronically. The fax number is 503-373-7493. Printed applications can be mailed to:
