[Libs-Or] services for adults survey questions

I am interested in surveying our adult users to get a better sense of which programs and services most interest them.  Has your library done a survey of this kind and would you be willing to share the questions you used and why you chose those questions?  Were you able to reach non-library users and if so, how did you do it?  Thanks!
