[Libs-Or] Samples of patron surveys addressing hours?

Hi folks,

We're looking at our reopening as a way to look at our usual schedule of hours open with fresh eyes, and I'd like to create a patron survey asking folks when they most commonly visit the library, if there are times the library is closed when they wish it were open, etc.

Has anyone created a similar survey that I could borrow from? I have some ideas on how to construct it, but I'd rather not reinvent the wheel!
