[Libs-Or] program waivers?

Okay next question...do any of you use waivers for programs with more risk (thinking X-acto knives, yoga, etc.)? Our city has one, but it's pretty heavy on the doom and gloom and you will definitely get injured, suffer death or dismemberment in this program, please sign this!

Hoping for some friendlier, but still legal-approved language. Any samples appreciated! Thanks.
