DMV would appreciate your comments on the draft Medical Referral Form for
the new medical reporting system. Click on the link below to view:

1.) Is it necessary to know if the impairment is acute, transient, chronic
or progressive?
2.) Are the instructions clear, and will you be able to complete the form
with minimal effort?
3.) Are the Impairment Definitions on the second page complete and accurate?

For purposes of sharing information under HIPAA, DMV is considered a public
health entity. The medical information contained in the report required by
DMV is confidential and shall only be used to determine the qualification of
a person to safely operate a motor vehicle, and for taking any action deemed
necessary by DMV.
