Dear Members:
It has been almost a month since DMV implemented the new mandatory medical
program and all is going well. There are a few things that you could do to
assist DMV with processing the Mandatory Impairment Referral form:
1.) Include underlying diagnosis/condition, date of last exam, and use the
text box to describe how the patient is affected by the impairment. If
attaching chart notes, you do not need to write in the box.
2.) If faxing the form to DMV, you do not need to mail a hard copy.
3.) Please make sure the impairment(s)) being reported meet the definition
of severe AND uncontrollable. Severe means the impairment substantially
limits a person's ability to perform activities of daily living, including
driving. Uncontrollable means the impairment cannot be controlled or
compensated for by surgery, medication, therapy or use of a driving device
or technique.
Please note: severe and uncontrollable DOES NOT INCLUDE a temporary
impairment for which the person is being treated by a physician or health
care provider, and which is not expected to last more than 6 months.
4.) It is no longer mandatory for physicians or other health care providers
practicing in your geographical area to report any single episode of loss of
consciousness and control until the impairment/condition becomes severe and
uncontrollable. Use of the Report of Disorders Affecting Consciousness Form
(# 6834) is no longer necessary.
