[MyOEBB] Members being moved to Non-Healthy Futures plans retroactively effective 10/1/2015

Dear Business Managers, Superintendents and Benefits Administrators:
During the recent Open Enrollment period, members were able to enroll in a Healthy Futures medical plan and begin receiving the Healthy Futures incentive (reduced deductible or copays) effective October 1, 2015, if they and their enrolled spouse/partner (if applicable) committed to complete their medical carrier's online health assessment by October 15, 2015. When members enrolled in this option, they acknowledged that if they did not complete the health assessment requirement by October 15, 2015, their plan would change back to the regular non-incentivized plan retroactively effective October 1, 2015.
This month, OEBB will be making these retroactive plan changes and notifying the affected members in two batches, by medical carrier:
Kaiser Members
Members who enrolled in a Kaiser Healthy Futures plan and did not complete the health assessment requirement by October 15, 2015, will have their plan retroactively changed to the regular non-incentivized plan this week. OEBB will mail the attached notification letter (all hard copy USPS - no email delivery) to these members today, November 4, 2015.
If your entity has any affected members in Kaiser plans, you currently have a list of these members in your document management area of MyOEBB. The file is named "Missing_HA_Report_103115".
Moda Members
Due to technical difficulties experienced by some Moda members attempting to complete their health assessment, the retroactive plan changes for Moda Members will take place mid-late November. The Moda report of affected members will take 2 - 3 additional weeks to process, and members who logged into their MyModa account attempting to complete their health assessment will be credited as having completed it, even if they were unable to do so. When Moda has finished processing, the list of affected Moda Members from your entity will be deposited into your document management area and we will send a follow-up message to notify you. OEBB will mail the attached letter to the affected Moda members 2 - 3 days later.
