The Board of Psychologist Examiners has not yet received the following in order to issue your 2010-2011 biannual license as of this date:

Signed Renewal Notice/Affidavit
Renewal Fee

Remember, a $200 Delinquent Fee is assessed if your renewal fee and/or CE report is not received, is deficient, or postmarked after January 1st.  If this postcard has crossed in the mail with your submission, please disregard.
