Dear Licensee,

This is a reminder to submit your completed Continuing Education Tracking Tool & Audit Report and all supporting documentation for activities claimed during your reporting period of January 1, 2010 through your birth month 2012.

If you have misplaced your form, you may download a replacement copy here: http://www.oregon.gov/obpe/Renewals_CE/CE_Audit_4-12.pdf

All documentation must be received in the Board's office by June 17, 2013.  If you have already mailed your materials, please disregard this message.
