Changes in Lab Reports/Sample Data Sheets

This message is intended only for individuals who deal with the ODOT
Material Laboratory Reports and Sample Data Sheets (form 4000).
Effective Tuesday July 1, 2008 the final laboratory report will no
longer have a signature stamp.  Instead the final laboratory report will
have "Kevin Brophy - Laboratory Services Manager" or "Keith Johnston -
Structural Services Engineer" appearing on the lower section of the
report.

Effective immediately I request that all Sample Data Sheets (form 4000)
include the Quality Control Compliance Specialist's name, contact
number, and certification number in the "REMARKS/SPECIAL REQUIREMENTS"
section of the form.  This will allow us to more effectively address our
customer's needs.  

This is a reminder that all Sample Data Sheets (form 4000) include the

number, and certification number in the "REMARKS/SPECIAL REQUIREMENTS"
section of the form.  This will allow us to more effectively address our
