25 cents per copy at archives only

Arizona have them online FREE
Missouri have them online FREE at least up to 1924 as of last week.
Any after 1924 to 1955 are available for $1.00 per certificate up to 5 
certificates per process before you can order another 5. However they 
become free as they add more death certificates online.

Note this announcement from Missouri:
Images to 1925 will be released by the end of July 2006, and we 
anticipate having images up to 1930 available in September 2006.
