Our daughter applied for a passport a while back (to do volunteer dental 
work in Guatemala) and low and behold, her name was wrong ("Janice" 
instead of Janie) on the required birth certificate -- my wife had even 
signed the original, but incorrect, birth registration card, but still, 
there it was -- Janice -- really, it was a bit of a hassle to get it 
corrected.

These things do happen,
