"Access to Records by Veterans, Next-of-Kin, or the Veteran's =
Representative." The point here is "Veterans and next-of-kin of =
deceased veterans have the same rights to full access to the record. =
Next-of-kin are the un-remarried widow or widower, son or daughter, =
father or mother, brother or sister of the deceased veteran." A request =
from this category can be done using the Internet but a "Signature =
Verification" form must be filled out and either FAX or mailed within 20 =
days.

=20

FAX NUMBER: 314-801-9049

MAILING ADDRESS: "NPRC WEB, 9700 Page Avenue, Saint Louis, Missouri =
63132-5100"=20

=20

NOTE: Due to routine processing this web application will not be =
available for use from 12:31 AM EST until 5:59 AM EST. Please submit =
your requests during the hours of 6:00 AM EST and 12:30 AM EST.

=20

=20

=20

If your status as "Next of Kin" is not defined above then you must fill =
out a "Standard Form 180". This form can either be FAX or mailed. This =
form can be obtained numerous ways. (1) From the WEB Site, (2) By =
writing to NPRC and asking for the form, (3) by FAX on demand, (4) From =
Department of Defense, (5) Federal Information Centers, (6) US Veterans =
Administrations Offices, (7) Local Veterans Service organizations, (8) =
State and County Veterans Offices

=20

FAX NUMBER: 314-801-9195

MAILING ADDRESS: "National Personnel Records Center, Military Personnel =
Records, 9700 Page Avenue, Saint Louis, Missouri 63132-5100"

=20

The minimum information needed is the veteran's Name used while in the =
military, Veterans SSAN, Veteran's Date of Birth, Place of Birth, Date =
of Death.

=20

Additional information that will speed up the search of a Veteran's =
records is: What branch of service, Dates of Service, Officer or =
Enlisted, and Military Service Number. Also was the veteran on "Active =
Service", "Reserve Service", or "National Guard"

=20

With nothing else but the basic Veterans information and using the US =
Mail system both ways it took 7 months to receive my Father-In-Law =
military records. His period of service was from July 1941 to Oct 1963.

Once I got these records I scan them (All 92 pages) and then went =
through them with a fine tooth comb. I discovered he had been married =
previously. His address when he entered the US Army, what his =
occupation was prior to coming into the military. His mother's address =
and his sister married name and address.

=20

These records also include all his awards and decorations. And a point =
I would like to make is that as the next of kin you can request a =
complete set of the Veteran's military decorations. Use the same =
"Standard Form 180" as you did to obtain the veterans records and =
sending it to the following address for those veterans who were =
discharged or retired from the US Military. =20

=20

"National Archives and Records Administration

"National Personnel Records Center

9700 Page Avenue

Saint Louis, Missouri 63132-1547

=20

Request and inquiries for reservists should be sent to:

=20

"National Archives and Records Administration

"National Personnel Records Center

One Reserve Way

Saint Louis, Missouri 63132-5100

=20

An acceptable statement for requesting a replacement of a Veteran medals =
and awards would be that the original medals and awards have been lost, =
stolen or were never issued. (Pick One). To expedite this process =
enclosed copies of any documentation you received when you got the =
veterans military records.=20
