NORTHWEST HEALTH FOUNDATION: The Community's Partner for Better Health

(1) Request for Proposals:  Preventing and Reducing Chronic Diseases through Policy using Community-Based Participatory Research (CBPR)

Northwest Health Foundation invites proposals that seek to reduce the burden of chronic diseases using CBPR approaches. The Foundation is particularly interested in prevention, health promotion, and disparities research that address social, environmental, and behavioral factors through policy and systems changes.  

This program will award three types of grants: partnership building, CBPR implementation and policy advocacy dissemination.  Award amounts will range from $10,000 to $180,000 for the total project, depending on the scale and duration of the project. Grant periods are 12-36 months.

(2)  Community Health Priorities:  Where does good health start? 

Is violence a health concern?  How can we stay healthy when chemicals in the environment make us sick?  Is social inequality a determinant of health? 

What will it take for all of us to be able to live in environments that sustain us and promote our well-being?  What does a healthy community look like, from your perspective?  

Community Health Priorities is a public health conversation.  Our goal is to engage people from all corners of the state to articulate a vision of a healthier life for everyone, and then develop policies that help us achieve that vision. (3)  NWHF and Oregon Department of Human Services (DHS)
Request for Proposals:  Access and Effectiveness Health Care Delivery Grant Program

NWHF, on behalf of DHS, is seeking proposals on innovative ways to improve access to, and the effectiveness of, health care delivery for families, in conjunction with House Bill 3626, Section 21.

(4)  Recent awards in Access, Physical Activity and Nutrition, Hatfield Fellowship and Nursing Workforce

Recent grants totaling $1,078,840 were approved by the NWHF board of directors in June.  Learn more at http://www.nwhf.org/award/grants_archive.php

Grant highlight:  NWHF contributed $60,000 to support the first national Geriatric Nursing Leadership Academy, sponsored by the John A. Hartford Foundation grant to the Sigma Theta Tau International Foundation.  The grant will support two additional Oregon nurses in the leadership cohort of 2008. Oregon will be the only state in the country with three Academy fellows in one year.  The goal of the GNLA is to prepare nurses already working in geriatric health care settings to be effective systems-change leaders.
