OSPS E-News ~ Processing IRS Levy Paperwork

We would like to draw your attention to a potential problem related to
the information that you complete on the forms that are sent to you in
an IRS Tax Levy.  When you are completing the employer portion, which
includes the levy acknowledgement and levy results, it is very important
that you enter nothing in Section 4 Levy Processing Information.  

 

Section 4 asks you to provide a proper mailing address for tax levies.
If you place your agency's address in that section, your reward will be
to receive the tax levies for each and every state agency!  

 

One state agency recently filled in their agency address.  It seemed to
them perfectly reasonable to do so.  They were a bit curious when they
began receiving all sorts of mail from the IRS that would normally go to
OSPS.  Our staff contacted the IRS and learned that this was the source
of a massive address change.  

 

We've been working with the IRS to get things changed back but wanted to
inform you about this so that you would be aware to not enter your
address in Section 4.  We would like to avoid a repeat of this change in
official mailing address.  Thanks very much for your assistance.
