Hello Payroll Professionals,

CHRO has updated the newly hired temporary employee offer of coverage worksheet to include a place for the OR number on the upper left hand corner.  Attached is a copy of the updated form for your reference.

The form will also be available on the webpage for the Affordable care act toolkit soon.  You can find the toolkit online at http://www.oregon.gov/das/HR/Pages/aca.aspx.  The updated form should be available online soon.
