UPDATE:  We have a new fax machine now.  If you have been using the alternate number you no longer need to.  503-378-3518 is the OSPS fax line.

The OSPS fax machine is currently experiencing technical difficulties.  We are asking agencies to scan their manual check requests if possible.  If you still need to fax a request,  please use the alternate fax number of (503) 378-2901.
