This is a final reminder that Exempt 2018 W-4 forms expire February 15, 2019.   Please review your E370-06-1 report that is available with your quarterly reports.



The 2019 W-4 is available on the OSPS website: http://www.oregon.gov/das/Financial/Payroll/Pages/forms.aspx



We are working on placing a link to the new Oregon Form OR-W-4 on our website under the payroll forms.  In the meantime, you can find it here: https://www.oregon.gov/DOR/forms/FormsPubs/form-or-w-4_101-402_2019.pdf



By February 15, payroll offices need to request a new Form W-4, Employee's Withholding Allowance Certificate, from each employee who claimed exemption from income tax withholding last year.



On February 16, any Form W-4 previously given to Payroll claiming exemption from withholding has expired and you should begin withholding by adding an entry to the P010 screen.  If the employee does not give you a new Form W-4, withhold tax as if he or she is single, with zero withholding allowances.  However, if you have an earlier Form W-4 for an employee that is valid, withhold based on the earlier Form W-4.
