[Self-Management_OR] Dec. 2014 Diabetes Prevention Program Leader Training and New Online Resources

Hi Everyone,

Please see below for announcements related to the National Diabetes Prevention Program (National DPP), a lifestyle change program proven to prevent type 2 diabetes in adults at risk for developing diabetes.

Interested in participating in a DPP Lifestyle Coach Training in December 2014?
Lavinia Goto of NorthWest Senior and Disability Services is working on organizing a DPP lifestyle coach training to be held December 2014 in Salem. In order for the training to be offered, at least 12 participants are needed. This would be a two-day training, on a Thurs-Friday or Friday-Saturday. The cost is $750 per person. (Through a special grant, the cost may be waived for participants from within the 12-county Eastern Oregon CCO region, including Sherman and Gilliam Counties where there is a need to identity potential DPP lifestyle coaches.) Statewide list of Diabetes Prevention Programs and related resources
A statewide DPP list is available on the OHA Public Health Division's "Self-Management - Take Control of Your Health" webpage at www.healthoregon.org/takecontrol<http://www.healthoregon.org/takecontrol>. Scroll down to the National DPP section of the page and you'll see a link to "Find a Diabetes Prevention Program in Oregon" which lists programs and contact information. There's also a link to online tools for prediabetes risk assessment, marketing resources for DPP providers, and links to diabetes self-management education programs (for referring people who have a current diabetes diagnosis).
