Outstanding Practice Award Nominations
As in years past, we will be presenting our Outstanding Practice in Self-Management Awards during the Annual Forum.

Please consider nominating a Master Trainer, T-Trainer, program coordinator, program leader or organization for an Outstanding Practice in Self-Management Award.  Recipients of the 2014 awards will be honored at the Self-Management Forum on November 19, 2014 at the Portland Hilton Downtown.

For more information, see the attached award nomination form (Spanish translation to the forms can be provided if needed).  Nominations are due to Susan Miles by close of business on Friday, October 31, 2014. The Self-Management Forum will be featured as an ancillary session on the morning of November 19.
Self-management programs help people living with chronic health conditions take charge by actively participating in their lives and in their healthcare.  Join the ninth annual gathering of stakeholders who support these programs across the state.  We'll share information, celebrate successes, and identify strategic opportunities to integrate self-management resources into Oregon's ongoing process of health system transformation.

If you plan on attending the Forum, you are encouraged to also register for the Pre-Forum Meet and Greet Networking session on the evening of November 18th.
Please note that if you are traveling 70 miles or more one way, your hotel stay for the night of the 18th will be paid for if you are attending the Self-Management Forum
