[Self-Management_OR] SRCH grant recipient announcement

 Dear Self-Management Network members,
The Public Health Division has recently announced that the Sustainable Relationships for Community Health (SRCH) grant has been awarded to five applicants. Each awardee is a partnership between one or more Coordinated Care Organizations, county public health departments, and self-management provider organizations.
SRCH Awardees (and partners)
AllCare CCO (Curry County, Jackson County, Josephine County, Rogue Valley Council of Governments)
Clackamas County Public Health Division (HealthShare CCO)
Deschutes County Health Services (Pacific Source Community Solutions Coordinated Care Organization)
InterCommunity Health Network Coordinated Care Org. (Benton County, Lincoln County, Linn County)
Lane County Public Health (Trillium Community Health Network, Lane Council of Governments)

During the spring and summer of 2015, SRCH grantees will participate in three facilitated discussions and technical assistance to engage leaders across CCOs, public health, and community-based organizations involved in health system transformation. The discussions will include local and national experts who will share content related to models of care for heart disease, diabetes and prediabetes.
 Institute participants will specifically address issues related to:

  *   Chronic disease care quality improvement, including use of quality measures, electronic health records and health information technology, and traditional health workers in team‐based care.
  *   Increasing the use of evidence-based community self-management programs through health system referral and reimbursement.
The SRCH Institutes will support community partnerships in developing formal commitments, such as memoranda of understanding and data-sharing agreements, to reinforce collaboration and a long-term commitment to health system improvement and community-clinical linkages.
