Dear Oregon Self-Management Network partners,

You are cordially invited to attend the Place Matters conference in Portland in October! All of the sessions are related to community-based supports for chronic disease self-management, because healthful environments support active self-management. Some of the sessions are pertinent to self-management program delivery (check out the Community-Clinic linkages breakout, highlighting successes from the Sustainable Relationships for Community Health/SRCH project that's building self-management referral and payment systems with Medicaid Coordinated Care Organizations).

I would especially encourage you to consider doing a poster presentation -further information is available on the registration site. I look forward to seeing you there!

The Oregon Health Authority-Public Health Division cordially invites you to Place Matters 2016 happening October 4-6, 2016 at the Oregon Convention Center in Portland.

The conference is a biennial gathering of people focused on addressing the leading drivers of chronic disease and health care costs: tobacco use, obesity, poor nutrition and physical inactivity. Conference sessions focus on changing policies and systems that create places where healthy options are in reach for all.  The conference attracts 400+ attendees from diverse sectors, including health systems, public health, transportation and land use, education and community-based agencies.

Nationally recognized speakers will touch on some of the key strategies that Oregon can use to combat the human and financial cost of chronic disease in our state. Keynote speakers include:

  *   Lynne Saxton, Director of Oregon Health Authority
  *   Soma Stout, MD MPH, Executive Lead - Health Improvement Institute and Cambridge Health Alliance, and executive lead of the 100 Million Healthier Lives campaign
  *   Luis Rodriguez, Oregon Director of Government Relations with the American Cancer Society Cancer Action Network
  *   Mark Pertschuk, Director of Grassroots Change, a leading organization on building movements for health, and former Executive Director of Americans for Non-Smokers' Rights
  *   Bob Gordon, Project Director, California LGBT Tobacco Education Partnership
  *   Shelina Foderingham, Director of Practice Improvement at the National Behavioral Health Network for Tobacco Cessation
  *   Robert Crane, MD, clinical professor of Family Medicine at the Ohio State University and founder of the Preventing Tobacco Addiction Foundation

This year, the conference also includes a poster session, an awards luncheon, and facilitated networking on emerging issues in public health.
