[Self-Management_OR] CDSMP and DSMP Leader Training in St. Helens August 2016

Hello Everyone:

The Public Health Foundation of Columbia County, Northwest Senior &
Disability Services, and HealthInsight are pleased to announce Leader
Trainings for the Stanford Chronic Disease Self Management  AND  Diabetes
Self Management Programs.  Here are the details:

 

Class:              Chronic Disease Self-Management Leader Training

Dates:            August 16, 17, 18, & 19  2016   

Times:            8:30am-4:30pm (lunch on own)

Place:             Columbia County Soil & Water Conservation District
Meeting Room

                        35285 Millard Road

                        St. Helens, OR  97051

 

Class:              Diabetes Self-Management Leader Training

Dates:            August 23 - 24, 2019

Times:            Day 1:  8:00am-12noon

                        Day 2:  8:30am-4:30pm

Place:             Columbia County Soil & Water Conservation District
Meeting Room

                        35285 Millard Road

                        St. Helens, OR  97051

 

Please find attached the flyer about the trainings as well as an
application for the DSMP and one for the CDSMP Leader Training.  Both must
be completed if you want both classes.   If you have any questions, please
