[Self-Management_OR] Need DSMP Leaders

We are currently working on a contract under the Center for Medicare & Medicaid Services to increase Medicare beneficiary graduates from the Stanford Diabetes Self-Management Program (DSMP). To support this initiative, we are looking for a few more Leaders to contract with to teach the Diabetes Self-Management Program in metro area clinics and community settings who are partnering with us.
