Good morning,

Please see information in bold font below.  I want to get things rolling on recruiting participants for an upcoming CDSMP group leader training our organization will be hosting in the new year at our Woodburn location.  I'd like to gauge interest and get people signed up sooner rather than later. What: Chronic Disease Self-Management Program (Stanford University/Self-Management Resource Center model, also known as "Living Well with Chronic Conditions")

When: March 2-3 & 9-10, 2018; approx. 9am-4pm
