[Self-Management_OR] Participate in a Focus Group for Program Coordinators and Managers regarding Patient Engagement in Self-Management Education

You are invited to participate in an online focus group about your current perspectives, attitudes and knowledge regarding self-management education programs and the role of self-management delivery programs in engaging community members in self-management education and patient care.



Online Focus Groups will be held on June 7 & 8.



If you are a program coordinator or program manager for an Oregon-based self-management education program, you are invited to participate in an online discussion with other program coordinators from across the state.

After you register, we will contact you to confirm your participation and provide additional information regarding the online discussion boards. We ask participants to sign into the discussion boards once each day for 10-15 minutes during the two-day focus group.



About this initiative

The focus groups are part of a two-year Special Innovation Project grant awarded to HealthInsight by the Centers for Medicare & Medicaid Services (CMS). The purpose of the project is to



*         Capture promising practices and emerging models from communities that are working on integrating evidence-based self-management programs (ESMPs) into patient care through designing innovative closed-loop referral and funding models

*         Better understand how integrating ESMPs impacts the activated patient following completion of an ESMP

*         Help accelerate shared learning across communities working to achieve integrated sustainable systems for ESMPs



The products and tools developed from this grant will help to inform future CMS funding and support for improving systems that integrate the use of ESMPs. Your participation in these focus groups will help inform final recommendations for CMS.
