[Self-Management_OR] Attrition Survey

Hello Everyone,

We are in the process of developing attrition surveys for those who didn't complete workshops and those who never made it to the workshop after registering. Do you have survey questions that have been "successful" to share? Or any other key lessons learned as related to data gathering around attrition. This effort applies across offerings in Spanish and English.
