[Self-Management_OR] Survey: Sustainability of Oregon's Self-Management Programs

On behalf of the Oregon Health Authority (OHA), Health Promotion and Chronic Disease Prevention (HPCDP) section, we are requesting your participation in a survey related to evidence-based and culturally appropriate self-management and lifestyle change programs. Your responses to this survey will help OHA better understand your organization's needs, and guide OHA in its work to further develop and sustain the necessary infrastructure for these valuable programs.

Your feedback is vital towards efforts to further develop and sustain programs such as the National Diabetes Prevention Program, Living Well with Chronic Conditions, the Oregon Tobacco Quit Line and more - all of which promote health, wellbeing and equity while reducing costs.

A few things to keep in mind:

  *   This survey will take about 30 minutes to complete. Once you press "done," your input will be sent directly to Coraggio, the Portland based consulting firm assisting OHA with this survey.
  *   All information that you provide will be confidential. Your answers will be used to generate a report that will be shared with HPCDP staff, with no individual response information included.
  *   Please complete the survey by Mon. March 25, 2019.
