This platform is currently powered by Unite Us, being sponsored by Kaiser Permanente and free for all Community Based Organizations, Community Health Clinics, and is meant to support referrals for anyone regardless of insurance. We want to encourage you to join the network that will provide you with the ability to: * List the eligibility criteria and how to appropriately access your DPP programs as well as other social service programs your offer in a secure, closed loop coordinated network * Receive referrals for your DPP programs from other Community Based Organizations and health systems * The ability to send closed loop referrals for your clients Social Determinant of Health needs that you do not provide * Data reporting and analytics of who you are sending referral and who you are receiving referrals from including the status of the referral and outcome Steps to join the network: 1. Complete the Partner Registration Form: This form requests information about your programs and staff to set up profiles in the Unite Us software and create user accounts. Complete the form HERE and refer to the partner registration guide and service types glossary as needed. Please call Unite Us for an in-person meeting to complete this form and think through your organization's workflows. For DPP programs please list them under the following categories Health> Chronic Disease Prevention & Management option. Please complete this form by November 22nd 2. Sign Up for Software Training (December 2-December 13th): Direct users of the software such as case managers, community health workers, referral coordinators, etc. are required to attend an in-person or virtual training to receive log-in credentials HERE.
