The Self-Management Resource Center is offering a PILOT training for new leaders to facilitate the Virtual Chronic Disease Self-Management (vCDSMP) workshop. This pilot training will be using Zoom video sessions.
This training is 7 weeks.  The first week has one session, and weeks 2-7 have 2 sessions each week.  Each session will be 2.5 hours. To complete the training, you are required to attend all 13 Zoom video meetings using your webcam and computer speakers.  There are homework assignments required before and during the training, so be sure you have the book and allow yourself enough time to complete the homework.

Leader Requirements for this training:

  *   Represented by a licensed organization
  *   Have your own stable, fast, internet connection
  *   Have your own computer and webcam (no mobile devices)
  *   Good sound quality
  *   Proficient in how to use your equipment

Training Details:
Dates: August 14 - September 25, 2020 (Tuesdays and Fridays)
Time: 10:00 am - 12:30 pm (Pacific time)
Fee: $500 per person (this is special pricing for this pilot only)

Registration is limited to 12 and is on a first come, first served basis.
We will accept only 2 from the same organization. If you have more than that they will be waitlisted.
Refund policy: 50% refund up to 2 weeks before the start date (August 14) and no refunds after the first session has met.
