We need your help to identify up to 10 clinics or CBOs who are committed to improving BP control rates and have a level of readiness to implement self-monitoring blood pressure initiatives.

  *   Education: Provider and patient education, including CME opportunities
  *   Equipment: Self-monitoring blood pressure (SMBP) cuffs
  *   Recognition: Participants are nationally recognized and awarded for their progress towards achieving blood pressure goals
  *   Peer Support: Opportunity to participate in quarterly trainings and peer-to-peer learning collaborative to discuss successes and challenges

The Healthy Heart Ambassador (HHA) Blood Pressure Self-Monitoring Program is a 4 month-duration evidence-based program consisting of one-on-one support and consultations as well as group nutrition seminars that helps participants to lower and manage their hypertension as well as learn about ways to eat healthier.

Oregon Health Authority will assist FQHCs and CBOs  to:

  *   Train Community Health Workers or other staff to become program facilitators to guide and encourage participants to find their own path to better blood pressure management.
