[Self-Management_OR] Funding opportunity for SMRC Program Delivery Organizations in Oregon

Hello Oregon Self-Management Partners,



Comagine Health invites you to submit a proposal in response to the attached funding opportunity for delivering the Self-Management Resource Center's (SMRC) Chronic Disease Self-Management Program (CDSMP), Tomando Control de su Salud, Diabetes Self-Management Program (DSMP), and/or Programa de Manejo Personal de la Diabetes.



Comagine Health is seeking to fund three program delivery organizations to schedule workshops and enroll a minimum of 32 participants within a 4-month timeframe. Three $5,000 contracts will be awarded to deliver programs between January 1-April 30, 2023.



Please submit a technical response and supporting documentation to CINO at comagine.org<mailto:CINO at comagine.org> by November 25, 2022. Information and details for the technical response are in the attached funding opportunity.
