[Self-Management_OR] Free DPP Lifestyle Coach Training for Umbrella Hub starts Dec 4

Hello colleagues,
Please see the attached flyer for a free National DPP Lifestyle Coach training that starts in early December. The training is for organizations who would like to or are already working with Oregon's first National DPP Umbrella Hub (Oregon Wellness Network). For those new to this work, background is provided below.
Background:
The National Diabetes Prevention Program (DPP)<https://www.cdc.gov/diabetes-prevention/index.html> is a year-long program proven to prevent or delay the onset of diabetes. Organizations across the country apply to the CDC to become a Recognized National DPP Organization where they send their staff to become trained Lifestyle Coaches who can offer the National DPP using a CDC-recognized curriculum. CDC-Recognized Organizations are required to consistently work with the CDC to report program results and to maintain their recognition status. During the course of the National DPP class series, data can also be entered onto Compass by the National DPP instructors (in a secure area) to allow for the completion of reports due to the CDC. For communities or organizations who want to offer the National DPP and have received their National DPP organization code but don't have the capacity to meet the rigor of becoming a fully CDC-Recognized Organization and/or to complete the ongoing reporting requirements to the CDC, Oregon has what's called a CDC Umbrella Hub Arrangement (UH or UHA). OHA-partner, Oregon Wellness Network, is the first CDC-recognized UH organization and can train Lifestyle Coaches and oversee the offering of programs - thus far this can be done for free. OWN also offers a paid service where it can bill medical insurance companies on behalf of organizations under their Umbrella Hub Arrangement.
